Do I need medical insurance if I join your plan?​
​ The annual Membership fee does not cover the cost of services covered by your health insurance. This may include the copay, deductible, and coinsurance of your respective insurance plans.

Do I still have to pay your annual fee if I am on Medicare?​
Yes. A membership fee is required to enroll in the Concierge Choice Care membership plan and the fee is NOT covered by Medicare. Under this plan, patients remain responsible for Medicare/insurer-required copays, co-insurance, and deductibles. Any services that are provided by us and are covered by Medicare or your insurance must be billed to Medicare or your insurance, per regulation because the physician accepts Medicare or is in-network with your insurance plan. Medicare should still pay for medical services obtained outside my office.

What are my options if I want to continue as your patient, but simply can't afford the cost?​
Although I have done my best to make this plan affordable, I realize that there will be some of you who will be unable to afford my annual fee. I have a plan to maintain a hybrid office that will allow you to continue seeing me but without the added value of the concierge services.

Will I receive a discount if I don't have my annual exam or use all of your services?
No. Your annual fee covers my availability for your medical care.
What happens if I move after I join?
If you must transfer your care, I will assist with finding a new physician. Your medical records will be sent promptly upon your release of information form completion. The balance of your yearly fee will be prorated and refunded.
